What a form can do
- Email the right person with a reply-to set to the reporter, and keep a CSV archive on the server.
- Post the report into EDDMapS through its API, as Colorado's public reporting form does.
- Collect weigh-ins for a volunteer competition and feed a live leaderboard.
- Take a photo of a paper field form and hand back a structured record, with a person confirming it before it is saved.
- Run without WordPress or a plugin to patch.
Where it runs
Sanpitch CWMA, Northern Utah CWMA and Summit CWMA each have reporting forms that reach their coordinators and keep an archive. The Colorado reporting form built for the Western Weed Coordinating Committee submits through the EDDMapS API. AgTag USA's intake reads a packing slip with AI and asks a person to confirm it.

- Will we get spammed?
- Every form ships with a hidden honeypot field, a minimum fill time, a per-address rate limit and link and content checks. Bots get a quiet fake success. If a program is targeted, we add Cloudflare Turnstile, which is free and invisible to most people.
- Where do the reports go?
- Wherever you say: an inbox, a shared mailbox, a CSV on the server, EDDMapS, or all of them.
- Can reports include a photo and a map pin?
- Yes. Photos are stored with the report and the location can be a tapped pin, a typed address or a nearest-trail description.
